We accept most major health insurance plans. If you’re unsure about which insurance we accept, please call us at 504-732-1094. Please note that Medicaid is not accepted as primary insurance.
Forms
New patients must bring the following information to their visit:
- Referral from primary care physician or specialist
- Drivers license/identification card
- Insurance card
You will be asked to supply some basic information such as your contact info, medical history, insurance, and financial liability.
Patients submitting a Worker’s Compensation claim will need to complete this form.
